SCHOLARSHIP COMMITTEE The committee consists of the five immediate Past Presidents of the OFDA. This committee sets the policy with respect to high scholastic standards required of scholarship beneficiaries. A maximum $1,500.00 scholarship will be awarded to persons who attend any accredited Mortuary College offering instruction in Mortuary Science. Only one scholarship will be given per recipient. The amount of the scholarship will be determined by the Scholarship Committee according to funds available. Payment will be made to the College on a semester or quarterly basis for payment of tuition and books ONLY. If requested by the Committee, recipient will make a personal appearance before the Scholarship Committee. The purpose of the Oklahoma Funeral Directors Association (OFDA) Scholarship Fund is to grant scholarships to young people of acute mind, pleasing personality, good character, ambitious purposes and positive qualities of leadership. QUALIFICATIONS 1. To receive this scholarship, the applicant must meet all the following requirements: A. Names of applicants must be presented by a member of OFDA. B. Recipient must meet the educational requirements of the Oklahoma Funeral Board. C. Residence Status – Applicant must be a legal resident of the state of Oklahoma for two years and must meet requirements of any accredited College offering instruction in Mortuary Science. D. Personal Qualifications – Applicant must possess an acute mind, a pleasing personality, good character, ambitious purpose and positive qualities of leadership. They must have abstained from participation in activities which created behavior incidents. E. When funds are awarded – 1. Funds to be used for an in-state mortuary science program will be awarded for use during the last two semesters of mortuary school. 2. Funds to be used for an out-of-state mortuary science program will be awarded for use after the applicant has provided proof of completion of 60 college hours and the student is eligible to enroll in the 4th quarter of the Mortuary Science program. F. Scholastic Requirements – Applicant must have earned at least a "C" average in his/her four years of high school. The applicant must maintain satisfactory work and good conduct while attending the College of Embalming and maintain at least a "C" average each semester or quarter to continue to receive scholarship payments. G. Information required to be completed before an application will be considered by the Committee: 1. A formal application completed. 2. The high school transcript showing grades earned and college credits. 3. Three letters of recommendation; two from business or professional people, and one from an OFDA member. 4. Evidence showing need of scholarship. 5. Recent photo or snapshot. 2. To retain a scholarship the following conditions must prevail: A. Recipient must remain in the same college where he has enrolled when the scholarship was granted. If recipient decides to change colleges and desires a scholarship at another college, the recipient must have the approval of the Scholarship Committee. B. Recipient must continue to exhibit the same traits and characteristics required of the scholarship. PAYBACK REQUIREMENTS I hereby agree in consideration of this scholarship loan upon completion of my course of study of Mortuary Science that I shall repay this scholarship by working in a funeral home in the state of Oklahoma for a period of two (2) years. I will seek employment in Oklahoma within a period of ninety (90) days, unless a longer period of time is agreed to by the Scholarship Committee and recipient. Any required service in the Armed Forces of the United States will be given consideration by the Scholarship Committee for extending the time needed to complete the two-year period of work in an Oklahoma funeral home. If I do not work in Oklahoma, I promise to pay back to the Oklahoma Funeral Directors Association Scholarship Fund the principal sum of the scholarship plus the passbook interest rate per annum from the date of disbursement to the College of Mortuary Science, plus all collections and attorney fees. OFDA will send a questionnaire approximately every six months that must be completed and returned to the OFDA office. Because of the Scholarship requirements, we must keep accurate records of your scholastic and professional progress from the time you receive the scholarship and for a period of two years after graduation.
