Membership Enrollment Form

UCO Students
All students in good standing at UCO are eligible for membership and access to the Wellness Center.

Alumni
Those UCO alumni that have paid fees toward the Wellness Center are eligible to receive membership based on the number of semester paid in since spring of 2000. Eligible alumni must present an UCO transcript upon membership application and enrollment.  

Faculty/Staff/Alumni Association member and Spouse
UCO faculty and staff and their spouses may purchase a wellness center membership with the following payment options:

Faculty/staff who choose the monthly payment plan must enroll in payroll deduction. Contact Leigha Joiner at extension 3150 to enroll in payroll deduction.

Faculty/staff who choose the 1-year or 6-month advance payment option may send their payment to: University of Central Oklahoma Wellness Center, 100 North University Drive, Edmond, OK 73034.

Student Spouse
Spouses of UCO students may purchase a Wellness Center membership with the following payment options:

All UCO Wellness Center members must be 18 years of age or older.

Community Member and Spouse

Retired UCO Employees
Free membership with letter from Hiring and Benefits.

Guests:
Members may bring a guest for $5 per visit (limit 2 guests per visit). Guests must be 18 years of age or older, be accompanied by a member, and sign a waiver of release and liability which is available at the front desk of the Wellness Center.
